1. Call 1-877-504-6672 (1-877-50-COMPANION)

2. Our Care Counselor will gather important information about your patient and family. Information such as:

  • Where is your patient being treated
  • When, if not currently, is your patient receiving treatment
  • Nature of the illness or medical procedure being done
  • Expected length of stay in the facility
  • Important contact names and numbers of patient and family

3. With our program being customized to the specific needs of each patient, we also ask questions to help us with the 'comfort' needs of our patients. Helpful input such as:

  • Patient's likes and dislikes
  • State of mind regarding illness or impending treatment
  • Interests and hobbies
  • Favorite movies, musical selections, books

4. Review of services, fees, contract and pertinent releases (HIPAA)

5. Suggested plan of care including recommendations for our Patient Enhancement Program and Family Support Program

6. Schedule a date and time for the service to begin.
